In 2025 they have lived together a total of 142 people at the Cedre socio-health center. The coexistence profile presents a distribution of 70% women and 30% menwith one average age of 83 and 78 years respectively. As specified in the memory of the SAASin addition, they have incorporated 35 new moves (income) during the year (22 women and 13 men), remaining at similar values to those of the previous year.
In relation to life outside the centre, they are accounted for 232 overnight stayswith a 20.5% decrease compared to 2024i 443 weekend departures, down 11.9%. These variations are relevant from one perspective of Person-Centered Care (CCP) because they can affect the continuity of social ties, roles, participation and supports to facilitate the connection of residents with the community environment.
The average daily departures of residents has been 1.5 users per daywhile others three people have taken a fifteen-day holiday away from home. During the year, they have also been organized 728 internal and 28 external activities (outings and events planned throughout the year).
Assistance activity and health care
The care activity consisted of an internal follow-up by the center’s medical area and in coordination with external resources. In this regard, they have registered 634 referrals to other medical specialties. In the field of interconsultations, they have been carried out 35 referrals to specialist doctors, 28 to psychiatry and 13 to rehabilitation. They have also been carried out 58 in speech therapy.
In addition, 3,267 visits have been made by the centre’s doctor, 107 physiotherapy interventions and 217 INEF interventions. This volume of interventions demonstrates the good interdisciplinary coordination and continuous communication that guarantees decisions aligned with the clinical needs and the person’s preferences, as indicated by the memory of SAS.
In the field of medical transporthave been offered a total of 431 services during the year 2025facilitating access to tests or specialized visits outside the center.
Psychosocial intervention
The Cedar ACP model it is also based on the importance of therapeutic, psychosocial and accompanying interventions. In this sense, during 2025 they have registered 270 psychology interventions, 680 in occupational therapy, 369 in social work and 419 in social education.
